Per your foster agreement all medications and supplements given to your foster dog must be logged. Please print this PDF and fill in the information accordingly adhering to dose and frequency prescribed. If you have questions with regards to prescription, administration or veterinary information reach out to your APW Director.

 

This form is vital in maintaining the best medical care and treatment of our rescued dogs and is required by law and our rescue license. We appreciate your diligence and detail with these records.
